Reflux and Safe Sleep: Why You Can't Tilt the Crib Mattress (And What Actually Helps)
Reading Time - ...
Does your baby cough, arch, and scream the moment you lay them down flat? Do they have reflux? Maybe someone has told you that propping up the mattress with a towel will help?
It was what my grandmother suggested for my own baby. I didn’t find out until I asked my pediatrician: tilting the crib mattress doesn't ease reflux, and it makes sleep less safe. Flat on the back is still the best choice, even with reflux. What really helps is what you do with the baby before bedtime.

Key Summary
|
Is It Safe to Elevate a Crib Mattress for Reflux?
No. The NIH Safe to Sleep campaign says raising one end of the crib or mattress does not reduce reflux. It's also dangerous because babies can slide down the slope into a position that makes breathing harder.
Why Flat on the Back Protects Your Baby

- On the back, the windpipe sits on top of the food pipe.
- Anything that comes up has to fight gravity to reach the lungs.
- Healthy babies have a built-in reflex to swallow or cough up fluids, even while asleep.
That's why the safe sleep guidelines call for a firm, flat, level surface for every baby, reflux or not.

What people say |
What the evidence shows |
|
"Tilt the mattress so milk stays down." |
Raising the crib or mattress does not reduce reflux symptoms. |
|
"Babies choke on their backs." |
Back sleeping has not increased choking. The airway sits above the food pipe. |
|
"A folded towel under the sheet is fine." |
Anything under the sheet can create slopes, gaps, and soft spots. |
|
"Side sleeping keeps spit-up out." |
Side and tummy positions raise SIDS risk, so they aren't advised for reflux. |
Why Do Towels, Books, and Homemade Wedges Make Things Worse?
A rolled towel feels like a small fix at 3 a.m. Here's what actually happens.
Your Baby Slides Down
Babies wriggle. On a slope, they slide toward the low end and can end up curled with their chin on their chest, a position that can block a young baby's airway.
The Surface Stops Being Firm and Even
A towel or book creates a ridge, a dip, or a gap at the edge. It can also pull the fitted sheet loose. A firm, flat crib mattress only works when nothing is pushing it out of shape.
What About Reflux Wedges and Tilted Bassinets?
The same goes for store-bought wedges. The CPSC and FDA warned parents to stop using sleep positioners, saying any reflux benefit is outweighed by the risk of suffocation.
Inclined sleepers used the same logic. Fisher-Price recalled all Rock 'n Play Sleepers in 2019, about 4.7 million units, after reports of infant deaths. Today, federal rules ban infant sleepers inclined more than 10 degrees.
Is It Everyday Spit-Up or Something More?
Reflux is very common. The NHS notes it usually starts before 8 weeks and gets better by around age 1. Most babies are "happy spitters": messy, but content and growing well.
A baby who cries through most feeds, arches often, or gains weight slowly may need a closer look.
What Actually Helps a Baby With Reflux Sleep Flat?

The real fixes happen during and after feeds, not on the mattress.

What helps |
When |
How to do it |
|
Upright time |
After every feed |
Hold your baby upright for 20 to 30 minutes before laying them down. |
|
Smaller feeds |
All day |
Offer a little less, a little more often, so the tummy isn't overfull. |
|
Paced bottle feeding |
Bottle feeds |
Hold the bottle more level and pause often so your baby controls the flow. |
|
Burping breaks |
Mid-feed |
Burp halfway through and again at the end. |
|
Pediatrician check |
If symptoms last |
Ask about feeding changes or a formula trial. |
Hold Upright, Then Lay Down Flat
NIDDK suggests holding your baby upright for 20 or 30 minutes after they eat, when you can. Then place them flat on their back to sleep.
Slow the Feed Down
The NASPGHAN and ESPGHAN reflux guidelines list avoiding overfeeding as a first step. Paced bottle feeding and regular burping help your baby eat slower and swallow less air.
Bring In Your Pediatrician
If simple changes don't help, your doctor may suggest thickened feeds or a 2- to 4-week formula trial, since cow's milk protein allergy can look like reflux. Only give medicine your doctor recommends.
How Do You Get Through Spit-Up Nights?
Reflux means more laundry. A little prep makes a 2 a.m. mess quick to fix.
- Keep 2 or 3 spare fitted sheets within arm's reach.
- Use a thin, snug waterproof protector under the sheet only, never on top. This mattress pad and cover guide explains what's safe.
- Never add burp cloths, towels, or blankets to the crib to catch spit-up.
- Wash on a gentle cycle so the elastic keeps its grip. Here's how to wash baby sheets without shrinking them.
The 60-Second Nighttime Reset
- Move your baby somewhere safe, like your arms or a second safe sleep space.
- Strip the wet sheet, wipe the protector, and pull a clean sheet on tight at all four corners.
- Lay your baby back down flat on their back.
When Should You Call the Doctor?
Call your pediatrician if your baby shows any of these warning signs:
- Isn't gaining weight, or is losing weight
- Has green or yellow vomit, or blood in vomit or poop
- Vomits with more force than usual
- Refuses feeds or has fewer wet diapers
- Starts reflux after 6 months, or still has it after age 1
Call 911 right away if your baby stops breathing, turns blue, or goes limp.
Flat, Fitted, and a Little Easier Every Week
Reflux is hard, but it passes. Until then, keep the crib flat, focus on feeds, and be ready for quick changes.
Joey & Joan fitted sheets are made from OEKO-TEX certified jersey cotton and sized to specific crib and bassinet models, so they pull on tight and stay flat, even on the fifth change of the night. Stock up on spare sheets before the next spit-up night.
Frequently Asked Questions
Can my baby with reflux sleep in a swing or car seat?
No. If your baby falls asleep sitting up, move them to a flat crib or bassinet as soon as you can.
Is it okay for my baby to nap upright on my chest?
Holding your baby upright while you're awake is fine. If you start to feel sleepy, lay them down flat in their own sleep space.
Does a pacifier help a baby with reflux?
Many babies find sucking soothing, and pacifiers at sleep time are linked to lower SIDS risk. Never attach one to clothing or a cord.
What is silent reflux?
It's when a baby shows reflux signs like fussing or gulping but doesn't bring milk up.
Why do babies get reflux in the first place?
The ring of muscle at the bottom of a baby's food pipe is still weak, so milk comes back up easily. It gets stronger as they grow.
